മുൻസിപ്പാലിറ്റി, കോർപ്പറേഷൻ ഒറ്റ അപേക്ഷ മതി

പഞ്ചായത്തിൽ വോട്ടുള്ളവർ മൂന്ന് അപേക്ഷ സമർപ്പിക്കണം

ഒന്ന്-പഞ്ചായത്തിലേക്ക് (പഞ്ചായത്ത് , വാർഡ് നമ്പർ & വാർഡ് പേര് )

രണ്ട് -ബ്ലോക്ക് പഞ്ചായത്ത്-(Block , Block ഡിവിഷൻ , പേര് )

മൂന്ന്-ജില്ല പഞ്ചായത്ത് (ജില്ല , ജില്ല പഞ്ചായത്ത് ഡിവിഷൻ പേര് )

ഫോറം 15 ലാണ് അപേക്ഷിക്കണ്ടത്

ത്രിതല പഞ്ചായത്ത് മൂന്ന് അപേക്ഷകളും ഒരു കവറിലാക്കി-പഞ്ചായത്ത്/ബ്ലോക്ക് പഞ്ചായത്ത് വരണാധികാരിക്ക് കൊടുക്കാം.നേരിട്ടും തപാലിലും കൊടുക്കാം Training സമയത്തും അവിടെ നൽകാം

മറ്റു ജില്ലകളിൽ വോട്ടുള്ള ജീവനക്കാർ അപേക്ഷ തപാലിൽ അയക്കാം.തിരഞ്ഞെടുപ്പിന് ഏഴു ദിവസം മുൻപ് കിട്ടണം.

പോസ്റ്റൽ ബാലറ്റ് കിട്ടിയ ശേഷം തപാലിൽ അയക്കാം.(അതിലുള്ള ഫോമുകൾ പ്രത്യേകം പൂരിപ്പിക്കണം)

അപേക്ഷയിൽ കാണുന്ന വോട്ടർ പട്ടികയിലെ ക്രമനമ്പർ-നമ്മുടെ സീരിയൽ നമ്പറാണ്.

വോട്ടർ പട്ടികയിലെ ഭാഗത്തിൻ്റെ ക്രമനമ്പർ എന്നു പറയുന്നത്-വാർഡിൻ്റെ നമ്പർ & ബൂത്ത് നമ്പർ . ഉദാഹരണം ഒരാളുടെ വോട്ട് വാർഡ് 10 ലെ രണ്ടാം ബൂത്തിലാണെങ്കിൽ 10/2 എന്നെഴുതണം. ചെറിയ വാർഡാണെങ്കിൽ ഒറ്റ ബൂത്ത് മാത്രം . അപ്പോൾ 5/1 ഇതു പോലെ എഴുതണം
